Cougar Health Services Eligibility Ends Upon Graduation
Once you graduate, you will no longer be able to receive care through the Medical Clinic, Vision Clinic, Pharmacy, or Counseling and Psychological Services (CAPS). To avoid any disruptions in your healthcare, we encourage you to finalize your care by following the steps below:
- Prescriptions: The options below will ensure you have refills available after graduation.
- Contact your provider or make an appointment. During an appointment, your prescription refills are updated and sent to your new pharmacy. Schedule an appointment.
- Transfer your prescription to a new pharmacy: Please verify that you have refills available to transfer. If you have refills, please contact your new pharmacy to start the transfer. You can find more information and steps by visiting the CHS Pharmacy webpage.
- Chronic Conditions & Ongoing Treatment: If you are managing a chronic condition or require regular medication, we strongly recommend finalizing your care plan now by scheduling a final appointment. Finding a new primary care provider (PCP) can take time, and early planning will help ensure continuity of care.
- Ongoing Mental Health Treatment: If you plan to continue therapy after your graduation, we recommend starting your search for a new therapist now. You can learn more about how to do that by visiting the CAPS webpage. If you have a prescription from a CAPS provider, we recommend you contact that provider now to ensure a seamless transition.
- Transfer Medical Records: If you would like to have your medical records transferred before graduating, please fill out the form in the patient portal or come in person to the Washington Building to fill out a form.
Note that the Medical Clinic and CAPS have different forms for transferring medical records. Additionally, CAPS record transfers are only available in person. Records will remain available at Cougar Health Services for 10 years.
