Cougar Health Services’ travel clinic can help you get immunizations before you travel and provide guidance on preventative measures you can take to stay healthy while you’re traveling, so you can have that life-changing experience without worrying about getting sick.

Because travel to some locations will require a series of immunizations, we recommend that you make your initial pre-travel consultation appointment at least 3 months before your expected departure date.
Before making an appointment with the travel clinic, please fill out our Travel Clinic Pre-Visit Questionnaire. When you’ve completed the form, you can upload the document in your patient portal, fax it to 509-335-6223, or print it and bring it to CHS. Copies of the form are also available at the CHS reception desk.
After submitting your form, please make your appointment through your Patient Portal or by calling 509-335-3575. Receiving your form in advance allows us to prepare for your appointment and make sure we cover everything we need to during your visit.
